Key Responsibilities:

  • Assessment: Evaluate clients' needs and strengths.

  • Case Planning: Develop personalized care plans.

  • Advocacy: Assist clients in accessing services and asserting their rights.

  • Resource Coordination: Connect clients with relevant community resources.

  • Crisis Intervention: Provide immediate support during emergencies.

  • Monitoring and Follow-up: Track progress and adjust plans as needed.

  • Collaboration: Work with other professionals to coordinate care.

  • Documentation: Maintain accurate records.

  • Education and Empowerment: Provide support and build skills.

  • Ethical Practice: Adhere to professional standards.

  • Cultural Competence: Respect diversity and ensure culturally competent services.

  • Qualifications

    Qualifications: To excel in this role, you should have:

  • Clinical Expertise: A bachelor's degree in counseling, psychology, or a related field.
