JOB REQUIREMENTS: Responsible for provision of case management services to homeless households in developing self-sufficiency skills and achieving housing stability. Primary functions of the position include partnering with participants to: assess strengths, needs and goals; develop an individualized service plan for meeting needs and achieving goals; refer and link to community resource providers; assess and implement services to build life skills; and evaluate follow-through and progress with meeting needs, achieving goals and program service requirements. Collaborate with the Program Manager and Program Director to develop and evaluate program effectiveness, including service delivery; provide recommendations on revisions based on research and evidence-based methods. Support and monitor participants in achieving action steps toward self-sufficiency goals identified in the Housing Stability Plan.
