Job Summary and Qualifications

  • Under the supervision of the Director of Case Management, the Case Management Manager performs case management functions; performs a leadership role by assisting the Director with the day-to-day operations of the department.

  • Acts as a resource and collaborates with the interdisciplinary team for the purpose of optimizing patient care and resource usage.

  • Provides daily leadership for case/resource management to achieve maximum deployment and integration. Measures such functions throughout the organization using the facility improvement process.

  • Utilizes data analysis to drive departmental and facility improvement projects.

  • Works cooperatively with physicians to achieve the utilization of the case management plan.

  • Collaborates with clinicians and support staff in designing and implementing processes to comply with regulatory requirements and assists with TJC survey readiness/preparation.

  • Participates in performance improvement activities of the department and organization.

  • Provides leadership for all areas of the RN Case Manager and Social Worker roles.

  • Assists with tracking trends for barriers to patient access to care; makes recommendations in the development of action plans to improve process and systems.

  • Demonstrates knowledge of regulatory requirements, HIPPA, HCA Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance as it relates to the Case Management Department roles.

  • Adheres to established policy and procedures and standards of care; escalates issues through the Chain of Command.

  • Actively provides and monitors training of all new departmental staff.

  • Assists the Director in the development and check-off of on-going staff competencies and ensures that all staff is in compliance with annual safety, HIPPA and Code of Conduct requirements; provides input on staff performance evaluations.

What qualifications you will need:

Education and Training Experience Required:

  • Graduate of an accredited professional school of nursing required; BSN or MSN preferred or graduate of an accredited professional school of social work, LMSW or LCSW required.

  • Two year’s recent experience in a similar leadership position and have a minimum of three-year’s experience as an RN Case Manager or SW Case Manager with responsibility for discharge planning and utilization resources.

  • Competent in data analysis; excellent verbal and written communication skills; knowledge of TJC and regulatory standards.

Certification/Licensure:

  • RN with current licensure or RN with temporary permit to practice from the Board of Nurse Examiners for the State of Texas; LMSW or LCSW with current SW license.

  • Certification in Case Management (CCM) or be able to obtain CCM within one-year of employment preferred.
