PositionSummary:

Under the supervision of the Coding and Data Quality Manager, is responsible for following established procedures for the review, classification, and abstraction of clinical data from patients’ medical records regarding diseases, treatment given, and operative procedures. Coder trainee abstracts and codes relevant data elements for each clinic, ancillary, outpatient surgery or inpatient discharge visit.

KeyResponsibilities include:

  • Utilizesthe patient management system (SER) to look up patients by name, phonetic andnumeric inquiry.

  • Obtainspatient information such as admission and discharge dates, clinicianinformation, and current location for in-house (admitted) patients.

  • Usesthe clinical information system (SAC) to obtain and print transcribed reports.

  • Usesterminal digit filing system when searching, filing, sorting charts andclinical documents.

  • Usescomputerized chart management and tracking system (SoftMed) to look up currentchart location, including chart history, reservations, and incomplete status.Creates and manages chart reservations.

  • Locatesmedical records upon request and charges record out to appropriate departmentin the computerized chart management and tracking system (SoftMed).

  • Checkscharts back into the department using the computerized chart management andtracking system (SoftMed).

  • Maintainspatient confidentiality as it relates to the release of medical information.

  • Fulfillsbasic release of medical information requests to other covered entities forcontinuing care purposes. Confirms admission and discharge dates. Assistscustomers to complete release of information consents.

  • Interactswith internal and external customers calling or walking into the department. Asneeded, directs customers (walk-ins and phone) to the appropriate person withinthe HIMS department. Handles all incoming calls.

  • Followsestablished City of Hope and department policies, procedures, objectives,performance improvement, attendance, safety, environmental, and infectioncontrol guidelines. Practices a highlevel of integrity and honesty in maintaining confidentiality.

  • Knowswhen to escalate issues to the appropriate department manager.

  • Performsother related duties as assigned or requested.

Coder Role Specific Functions:

  • Readsand interprets medical record documentation to identify all diagnosis,conditions, problems and procedures for inpatient discharge, outpatientsurgery, outpatient clinic and ancillary services coding.

  • Clarifiesconflicting, ambiguous, or non- specific information appearing in a medical recordby consulting the appropriate physician.

  • AppliesUniform Hospital Discharge Data Set definitions to select principal diagnosis,principal procedure, complications, co-morbid conditions, other diagnoses andsignificant procedures which require coding.

  • Appliesknowledge of ICD-9-CM instructional notations and conventions to locate andassign the correct diagnostic and procedural codes and sequence themcorrectly.

  • Appliesknowledge of current approved ICD-9-CM coding guidelines to assign and sequencethe correct diagnoses and procedure codes.

  • Appliesknowledge of anatomy, clinical disease processes, and diagnostic and proceduralterminology to assign accurate codes to diagnoses and procedures.

  • Appliesthe Basic Coding Guidelines for Inpatient and Outpatient Services to select and sequence diagnoses, conditions,problems, or other reasons which require coding.

  • Appliesknowledge of CPT coding guidelines andnotes to locate the correct codes for all services and procedures performedduring the encounter and sequence them correctly.

  • Appliesknowledge of payer reimbursement guidelines to ensure optimalreimbursement.

  • Abilityto utilize computerized encoder/grouper as a reference tool and for coding.

  • Keepscurrent with ICD-9-CM and CPT code changes, coding guidelines, and codingupdates.

  • Assistwith charge corrections as identified when coding outpatient encounters.

  • Reviewsand completes required reporting documents as required by external and internalsystems.

  • Completesweekly productivity reports and submits them to the manager.

  • Followsestablished City of Hope and department policies, procedures, objectives,performance improvement, attendance, safety, environmental, and infectioncontrol guidelines. Practices a highlevel of integrity and honesty in maintaining confidentiality.

  • Consistentlymeets coding quality standards and thresholds.

  • Attendsmeetings as required.

  • Successfullycompletes required education courses.

  • Performsother related duties as assigned or requested.

Basic education, experience and skillsrequired for consideration:

  • Post HighSchool or equivalent.

  • Thoroughknowledge of medical terminology/anatomy/ physiology/ coding principles.
