Description
Join Callen-Lorde Community Health Center, a leading LGBTQ-focused health center, as a Case Manager. We are seeking a compassionate and dedicated professional to support our diverse patient population in accessing comprehensive healthcare services.
Responsibilities:
Conduct comprehensive assessments of patients' needs, creating individualized care plans.
Collaborate with healthcare providers and community resources to ensure a holistic approach to patient care.
Advocate for patients' rights and coordinate access to LGBTQ-affirming services.
Facilitate support groups and educational sessions on relevant health topics.
Maintain accurate and confidential case records, ensuring compliance with HIPAA regulations.
Provide crisis intervention and emotional support to patients facing challenging situations.
Collaborate with multidisciplinary teams to enhance patient outcomes.
Qualifications
Education, Experience• High school diploma or GED preferred.• In the absence of education credentials, experience in social services, case management, and/or health care will be considered.• Familiarity with LGBTQIA, HIV and health-related issues and community resources.• Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.• Strong customer service orientation.
