Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Currently, we are seeking a full time Case Manager to work in our Homeless Services Division. This position has the responsibilities of providing case management and counseling to substance abusers who have been receiving treatment either in a residential substance abuse treatment program or an OSHA Clean and Sober shelter and are now seeking housing placement under a federally funded housing program, contingent upon eligibility. They also assess and identify a client's needs and appropriateness to participate in the program. Some of the responsibilities include the following: Assessing prospective client needs and eligibility to participate in the housing program and affiliated services. Providing case management for an assigned caseload of clients, (which may include their dependent children), comprised of individuals who are re-entering the local community and receiving housing through the Shelter Plus Care initiative. Developing an aftercare plan in tandem with the client and their respective counselor; arrange for placement and follow-up for services as specified through the aftercare plan. Ensuring individuals receive the benefits for which they are entitled, such as financial, medical vocational and education. Serving as an advocate and encourage clients to assume a proactive role in their rehabilitation and recovery process. Maintaining an on-going relationship with the local community and area service and housing providers to facilitate program cooperation and provide for constructive input and/or alternative resources to current plans. Emphasizing commitment and assistance to clients upon discharge or in the event of a relapse throughout the recovery process. Attempt to re-engage any client who departs from the Shelter Plus Care housing against program advice. Coordinating referrals with other agencies/facilities o meet the assessed needs of the clients.

Job Qualifications:

A.A. Degree in Human Services/Social Services, Healthcare or related field and a least two years of experience in substance abuse, or any combination of experience, education, or training that would provide the level of knowledge, skill and ability required.

Certified Addiction Counselor (CAC), preferred or on-going coursework in Drug and Alcohol Counseling to qualify for certification.

Very good communication skill, both oral and written.

Ability to work independently and as part of a focus team.

Detailed oriented.

Scope:

Frequent talking and listening.

Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with clients.

May require crisis intervention in emergency situations.

Possession of a valid driver's license.
