Hmong American Partnership Housing Stability Case Manager in Saint Paul, Minnesota

Primary Function of the Position Under the direction of the Director of Community Impact, the Housing Stability Case Manager is responsible for the coordination of HAP s homelessness prevention program, including and not limited to oversight of program outcomes, reporting and continuous improvement; building internal and external partnerships; and community outreach. This position ensures case management quality by adhering to agency policies and departmental procedures, managing related data collection and reporting. To fulfill this role the Case Manager must have good interpersonal skills; possess a working knowledge of homelessness prevention and finances; be an effective problem-solver; be able to effectively communicate in a constructive, nonjudgmental manner; be able to effectively support integrated programs as assigned; and must be organized and efficient in time and work management while being adaptable. Primary responsibilities include working closely with clients to provide direct assistance for renter households and homeowners that are at risk of homelessness, help participants access community resources, gathering and entering information into enrollment databases, making appropriate housing and other referrals, liaising with landlords, and program oversight. This position will provide in-person client assistance to support the Hmong, Karen, and Karenni Communities. Position Responsibilities Coordinate HAP s homelessness prevention program, including and not limited to case management, grant reporting and budgets. Provide homeless prevention case management to clients through financial and housing assistance and guidance. Support in developing strategies for addressing homelessness prevention objectives. Interact with clients and partners in a professional, nonjudgemental and positive manner. Advocate on behalf of the clients to legal aid/attorneys, benefit providers, and other resources. Develop and maintain strong relationships with community landlords and property managers. Ensure client files are complete, accurate and compliant, including obtaining appropriate information releases, file documentation, proper database entry, and related correspondence. Achieve all required evaluation and reporting deadlines for all funding sources. Maintain accurate records and reports of all program activities. Use evaluation data to improve program quality and effectiveness as part of a continuous quality improvement effort. Build and maintain collaborative relationships with internal and external partners and clients. Participate in staff and funder meetings and trainings. Coordinate and attend community outreach events. Other duties as assigned to accomplish program goals, outcomes and to support the agency s mission. Must have the ability to move about the office to access files and office equipment. Frequent moving of boxes and files weighing up to 20 pounds. Requires occasional bending, kneeling, balancing and reaching. Ability to focus on data, analyze the information and draw conclusions. Frequent interactions with coworkers, participants and partners requiring ability to communicate effectively. Frequent communication in writing and via email requiring ability to fluently read and write in English. Occasional preparation and analysis of figures.
