Northeast Valley Health Corporation Licensed Vocational Nurse II - E6 St Outreach in San Fernando, California

The Licensed Vocational Nurse (LVN) II provides a variety of administrative, clinical nursing, health education, and outreach services within the community health center and in the field. Basic skills for physical assessment (data collection) include observation, auscultation, palpation and percussion. Basic mental status assessment includes observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person and place, and assessment of recall ability. When the LVN notes a problem, complication or abnormality, he/she must notify the physician or RN under whose direction the licensee functions. The LVN cannot independently evaluate or synthesize data.

Reports to: Programmatic: Homeless Division Medical Director Professional/Technical: Registered Nurse (RN) Supervisor

Qualifications: 1. Must be a high school graduate or have a General Educational Development (GED) equivalency. 2. Satisfactory completion of a state approved program in vocational nursing, plus current license to practice in the State of California. 3. Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy. 4. A minimum of three (3) years experience as License Vocational Nurse in the Ambulatory Care with patient education expertise demonstrated by 40+ hours of Continuing Education Courses or trainings related to: a. HIV b. Special Population c. Diabetes d. Chronic Disease Management e. Substance Abuse or Mental Health f. Case Management g. High Risk Case Management h. Transitions in Care Case Management i. Comprehensive Perinatal Services Program Trainings j. Womens Health k. Approved training by Supervisor that relates to your current position 5. Must possess excellent interpersonal skills. 6. Must be willing and able to work a varied and flexible schedule to accommodate the needs of NEVHC and its patients. 7. Must be willing to travel to different outreach sites. 8. Must maintain or exceed competency levels as established by corporate standards for clinical practice. 9. Must demonstrate the knowledge and skills necessary to provide appropriate care. 10. Fluency in English (speaks, read and write). 11. Fluency in Spanish (speaks, translate, read and write). 12. Ability to maintain absolute confidentiality about health care and other patient/client information. 13. Current California drivers license, appropriate insurance coverage and a driving record acceptable to the NEVHCs insurance carrier (if required to drive on the job). 14. The MIS Patient Information System access level is 2.
