Use your administrative and social services skills as a Case Manager! The Salvation Army is seeking a Case Manager to provide trauma-informed care and housing-focused case management for individuals experiencing homelessness, with a focus on those with co-occurring disorders. This includes developing individualized service plans and providing intensive case management to support self-sufficiency. This position is full-time, non-exempt, and located in Redmond, Washington. Pay rate: $28.45/hour ESSENTIAL DUTIES & RESPONSIBILITIES: Provide comprehensive case management services including screening, assessment, individual service plan creation, crisis intervention, and linkage to community resources. Offer case management support for residents facing barriers to economic independence. Supply information on services and offer transportation assistance as needed. Continuously monitor and evaluate resident progression through case management plans, revising as necessary. Assist residents with Social Security or State Assistance applications, requiring SOAR training within 6 months. Serve as a mentor to Case Manager Assistants (CMAs), providing consultation and support. Assist with case file reviews, quality assurance, and compliance with funding requirements. Aid in resident assignment and tracking with the Assistant Director. Actively manage program funds in partnership with the Assistant Director and Resource Coordinator. Assist in developing aftercare Housing Stability Plans and facilitating resident exits from the program. Maintain regular contact with caseload, transitioning to semi-monthly meetings after 30 days. Assist in LGBT education and resource development for staff, including health issue identification and referral guidance. EDUCATION & EXPERIENCE: Bachelor in Social Work or related field preferred or equivalent experience Experience in 3 or more of the following areas: human services, case management, housing navigation, community building, and knowledge of resources for individuals experiencing homelessness with co-morbidities. Experience working with individuals experiencing homelessness in shelter or residential programs preferred. Experience working with individuals with high acuity needs due to behavioral health or other factors preferred.
