This position will coordinate care for patients in several remote locations for chronic care appointments, follow up for recent hospitalization, and an advocate to patient needs. Must be culturally sensitive, respectful and courteous. Strong organizational skills, communication skills to care providers and patients, proficient in computer, telephone communication a must. Experience in Care Coordination/Case Manager required.
