Trinity Health Social Worker, Case Management - 24 hrs in Waterbury, Connecticut

Position Purpose

St. Mary’s Hospital is looking for Social Worker to join our mission driven Case Management team. The medical Social Worker will work collaboratively with patients, families and the medical team to ensure patients recover and achieve optimal well-being.

What will you do:

  • Preform psychosocial assessments to determine patient care needs.

  • Helps connect patients to resources within the community.

  • Provides direct Social Work services to patients/families throughout the hospital setting.

  • Assesses patient/families for Social Work intervention upon admission & throughout discharge.

Requirements :

  • Education: MSW degree from an accredited School of Social Work required.

  • CT State Licensed Master of Social Work (LMSW) or CT State Clinical Social Work Licensure (LCSW) preferred.

  • Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.

  • Two to five years of diversified, progressive experience in acute care and/or other settings within the continuum preferred.

  • Experience with utilization management, home care, community resources, and rehabilitation beneficial.
