Cluster Community Services Mica Case Manager - Mental Health Progra in Yonkers, New York

Our ideal candidate will have experience in and be able to:

Develops and implements individual rehabilitative service plans with residents. Carry a case load of at least two (2) clients, as well as record restorative services for all SAS goals. Develop and implement individual rehabilitative service plans with residents. Provide training and education in areas related to chemical dependency. In addition to training new staff. Develop, implement and conduct a variety of structured group experiences toward maintaining and/or developing skills to increase functioning in the environment and the community; and to gain control over substance problems. Meet with designated MICA clients in other CLUSTER divisions as needed and consult with other staff on MICA related issues. Provide Restorative Services to residential clients. Provision of these services may be required through the use of individual or group interventions or by referral to appropriate community resources. Maintain all required written records, specifically including but not limited to individual case records, medication, shift logs and communication books. Follow all programmatic policies and procedures related to Administrative, Emergency and Direct Service practices. Follow all applicable laws and regulations including but not limited to those regarding provision of Medicaid Restorative Services, Health Insurance Portability and Accountability Act (HIPAA), OSHA. Maintain appropriate level of training including first aid and other items deemed necessary. Attends and participates in required meetings. Participate in maintenance of physical environment (House or Apartment), and support clients in doing the same. . Maintains collaborative contacts with other individuals and agencies providing services to residents in order to ensure consistency in treatment planning and service provision. . Provide assistance as necessary to the Program Coordinator with overall management of the program, including coverage in the absence of the coordinator. ' Performs other, related duties as necessary and/or assigned.

Who you will report to. The Program Coordinator- Congregate Residence

Education and experiential requirements.

  • New York State Credential as Alcoholism and Substance Abuse Counselor (CASAC) preferred, or

  • CASAC-T (In training), considered; (internal supervision will be provided)

  • Bachelors Degree in a discipline of social science and experience in the field of Human/social Services.

  • At least 1 year of experience working with MICA clients (preferred, but not required).

A valid New York State Drivers License is Required. *This requirement is not negotiable.
